Boy Scouts requires each scout to have a Medical Information Form on file. This is so the leaders have access to your son's medical information in case of a illness or injury during a Troop activity. These forms need to be updated each year as they are only good for 12 months.
Boy Scouts requires that for any overnight activity of 72 hours or less, only Parts A and B need to be completed – i.e., you don't need a doctor to complete Part C. However, for summer camp (which is more than 72 hours), Part C must be completed.

Note: This document contains all of the medical forms that are available from the BSA, but the only parts that are important to us are A, B, and C.
